Just my 2 cents but sounds more like vacuum leak, maybe intake gasket. you can spray some carb cleaner around the intake and other vac lines with the engine idling, and if it picks up idle, then you know where your leak is. Also, you mentioned that on cold starts idle was fine. but after grtting warm you began having issues with it. If the intake is leaking it would idle worse after warming up due to heat expansion making the leak worse. during braking it stalls, sounds like vac leak. Of course i could be WAY OFF! Good Luck to you.
if stalling when braking, it could very well be the IAC. mine is doing a lot of the same things you're saying yours is doing, and i've replaced the TPS, IAC, and PVC valve to no avail. still sporatically stalling and sputtering . i donno what else to do, so i guess i'm gonna have a mechanic take a look at it *sigh*

I have done a bit more diagnosing the problems symtoms, and I found the RPM's fluctuate slightly when I change from PARK, to REVERSE, to NEUTRAL, to DRIVE. The RPM's are also only really low when I have it in DRIVE or REVERSE and it is idling. Sounds like a IAC maybe? Would that help determine the problem?
that is normal, the subie does that, (its an auto) because you are pushing fluid through the transmission when it is in gear, plus you are turning the gears in the transmission.
